| Description: | Responsible for managing a multidisciplinary team. Answers the patient's and field staff's questions and assists with problem solving. Improves the quality of patient care by coaching and intervening to facilitate patient progression. Ensures optimal, clinical, financial, and satisfaction outcomes are achieved. Some of the responsibilities include selection and retention of staff, completing performance evaluations, mentoring and coaching the staff, corrective action as needed, assisting with audits, and acting as a preceptor. · Acts as a resource and has a working knowledge of Medicare, Medicaid and private insurance guidelines related to admission/service criteria and its proper clinical documentation. |
| Qualifications: | Must possess a degree in nursing (RN), physical therapy, speech/language pathology or occupational therapy with a valid and current NC license. Must have adult assessment skills. Requires three years of recent clinical experience, one year of community/home health experience and/or supervisory experience. |
