Admissions Care Manager - High Point, NC

The Regional Admissions Care Manager will assist the Regional Clinical Manager with day to day activities of the team and serve as backup in the Clinical Manager's absences.  The Care Manager will be available to the patients as well as to the field staff to answer questions and assist with problem solving so that the patient's needs will be met in the most appropriate and timely manner.  The Care Manager improves quality of patient care by coaching and intervening to facilitate patient progression through the plan of care.  The Care Manager works with a high degree of independence to ensure that optimal clinical, financial and satisfaction outcomes are achieved in a timely and efficient manner.  The Care Manager serves as a catalyst to challenge current practices and streamline delivery of services.

This position manages the Admissions and Triage team for the region.  They are the point of contact for all region admissions operations, handle the staffing requirements for the field Admissions RNs and assist with marketing and the intake end of referrals.  They partner with our coordinators in the hospitals to ensure patient satisfaction during their discharge at the hospital.
