| Description: | We are looking for an enthusiastic, self-directed individual to join our team as a Home Care Coordinator! Responsible for ensuring continuity between the hospital and the home setting in order to maximize patient satisfaction of home health services. This position is a sales/service position, handling new HH referrals at High Point Regional hospital & local physician offices. This is a 32 hour a week position. |
| Qualifications: | The Home Health Coordinator shall possess a two year degree in nursing from an accredited or approved school of nursing plus one year of healthcare or sales experience is required or one year nursing/case management. The candidate must have a valid current registered nursing license in the State of North Carolina. Excellent verbal and written communication skills as well as strong organizational and time management skills, problem solving skills and flexibility are required. |
