The Home Care Specialist is responsible for ensuring continuity between the hospital and the home setting in order to maximize patient satisfaction of home health services; to optimize patient safety, comfort, and well-being; to improve awareness and confidence among healthcare professionals, physicians and patients regarding current home health options and capabilities; to coordinate referrals to ensure timely admission and appropriate patient care based on doctor's referrals.

Duties:

This position will have a territory in Abingdon, VA, Bristol, TN, Bristol, VA as well as some additional areas of Southwest Virginia

Qualifications:The Home Care Specialist shall possess a 2-4 year college degree and 4 years work related experience in customer service, retail sales, or clinical exp.  Previous work experience in the health care field is preferred.  Healthcare sales is a plus.  Candidate should demonstrate outstanding customer service, broad base knowledge and have demonstrated leadership potential.  Candidate must possess computer skills, strong organizational, time management and problem solving skills, flexibility and the ability to work independently. Requires valid Driver's License.
